Vantage Healthcare takes the comprehensive office-based primary care practice mobile.  We bring our primary care team onsite for residents of Assisted Living communities and Nursing Homes, post-hospital rehab patients in Skilled Nursing Facilities, and patients post-hospital at home.

Our physician practice is among the most experienced, high-performing physician practices in care for seniors.  Our practice visits on-site at assisted living, skilled nursing and other senior living communities and settings and lead integrated teams of nurses and rehab therapists at each location.
